We are pleased you are considering participating in baseball at Bellevue High School. We have developed this handout to help familiarize you with Bellevue High School’s eligibility expectations of its student-athletes.
The eligibility process begins when a student and their parent(s) fully completes a Co-Curricular Athletics Participation Folder. The Co-Curricular Athletics Participation Folders (sports packets) are available in the Bellevue High School Athletics/Activities Office.
Tryout Dates / Times
- DATE: Tryouts begin March 1st, 2010
- TIME: 3:30pm – 6pm
- WHERE: Bellevue High School Baseball Field
- BRING: Full baseball gear/attire (glove, bats, cleats, baseball pants, long sleeves, and hat)
Participation Checklist and Eligibility Requirements (*Multiple Sports Athletes in Bold)
- *Completion of the Bellevue High School Emergency Information Card / Repeat Eligibility Information Form and provide proof of insurance coverage (If you are doing multiple sports you will have to complete this form for each sport prior to each season.)
- *Provide current physical form signed and dated by a physician (Good for 24 months or 6 sport seasons within two years if the physical is dated and completed after June 1)
- *Provide proof of adequate private family insurance or enroll and pay for the Bellevue School District insurance plan
- *Complete a Bellevue School District Parent Athlete Concussion Information Sheet
- Purchase a Bellevue High School ASB Card and pay all fines and fees
- Complete and sign every portion of the Bellevue High School Co-Curricular sports packet folder that applies
- Review the Bellevue School District Substance Use Form (Athletic Code) and sign the section in the sports packet that states you have done so and will abide by the Bellevue School District and WIAA rules
- Students transferring into Bellevue High School or attending a private or alternative school must meet with the Athletic Director and provide two proof of residency, an academic transcript and progress reports
- Athletic Directors completes a grade check; Grade Check Dates: February 9, March 23, April 20, May 18
